Something wrong with the keys of the parent tree, the number tree that maps each page's StructParents and each annotation's StructParent back to structure elements. A reader looks keys up assuming they're sorted and unique, so it can miss entries.

A key appears more than once; PDiffy reads only its first entry.

UnsortedKeys = 1

A key comes after a larger one.

NextKeyTooLow = 2

ParentTreeNextKey, the key a tool gives the next content it adds, isn't larger than every key in the tree, so new content could take an existing key.
